1 - Verify Eligibility
• The application must be completed with the subscriber present.
• Applicant must provide: full name, address, and date of birth. Provide last 4 digits of the SSN if possible and qualifying program(s) participation (e.g. Medicaid).
• Applicant must initial and e-sign the application and consent forms.
• Once the applicant is verified DO NOT CLOSE BROWSER! You need the application ID for Step 2.
• All information provided needs to be complete and true.
