Verify that you are eligible…

You will be asked for info to verify your identity:

• Name
• Date of birth
• Address (make sure to enter it exactly as it appears on your government ID, for example 5 Maple Ave instead of 5 Maple Avenue)
• The last 4 digits of your social security number
• Whether you participate in any benefit programs (for example Medicaid, SNAP, EBT, student grants, school lunch).
